This most likely occurs because your server is stuck-sub.
I'll explain what is happening here. Error indicates that mentioned effect or skill data has already been saved for the same subclass of that player.
Depend on how much you have customized subclass system, here might be multiple causes regarding this.
One example is a player having 2 identical subclasses.
